TEAM (Together Everyone Achieves More) ٍٍٍ½خر Home Training

The American Academy of Pediatrics defines ٍٍٍ½خر Home as accessible, family-centered, continuous, comprehensive, coordinated, compassionate and culturally competent. A core outcome of Healthy People 2010 and many national organizations is that all children have a ٍٍٍ½خر Home. Multiple surveys indicate that physicians do not feel residency prepared them to translate the philosophy of ٍٍٍ½خر Home into an effective systems based practice for Children and Youth with Special Health Care Needs (CYSHCN).

To address this, the ٍٍٍ½خر, Children’s Wisconsin and the Southeast Regional Center for CYSHCN developed TEAM ٍٍٍ½خر Home Training, a required curriculum spanning all three years of pediatric residency. TEAM improves health and quality of life by preparing future physicians to effectively provide ٍٍٍ½خر Homes to CYSHCN. The emphasis is on systems based care and the coordination of community based, tertiary and primary services focused on meeting not only the child’s medical but also their psychosocial, educational, recreational and emotional needs. Year 1 and 2 residents are introduced to ٍٍٍ½خر Home, CYSHCN, and family impact though: home visits, discussion groups, noon conferences, and workshops. Year 3 focuses on translating this into practical application during a block rotation.

Bridge to Independence

Bridge to Independence Program Bridge to Independence is an educational program for families of children with special health care needs. The goal to help families gain care coordination skills that will make it easier to work with the health care system.

Bridge to Independence has 12 modules, each covering a separate skill area. They include basic information about the skill area, actions to take, information about further resources, and forms to help organize or keep track of information.
